What should I do with an incorrect EB-5 case number?

My EB-5 I-526 was filed in Sept. 2018 with a receipt number ending in 99. In Dec. 2020, I received an RFE with the receipt number ending in 98. I caught this after responding to the RFE and sent a letter to USCIS letting them know about this. In March 2021, my original case number ending in 99 got approved, but I did not receive any approval notice, even after following up several times. After the Regional Center program was reauthorized, IPO asked me to file an I-824 duplicate approval notice. I just got the notice and it looks like number 99 has a different petitioner name. Does this mean my case number was changed to 98 automatically? How do I find out? What should be my next steps?

You should not have to file an I-824 that takes forever to adjudicate to get an approval notice. Also, your lawyer should have received a copy of the original approval. Getting the file to NVC is a separate issue and for this you have to ask IPO to forward your file. Sadly, we have had to file mandamus actions merely to accomplish this since IPO is non-responsive in most cases.
