You need a consultation appointment with an immigration attorney. Your questions raise different issues.
You do not need a lawyer to file an application/petition with USCIS - however, it is strongly recommended to retain counsel to avoid unnecessary delays and/or errors in the application process.
You can file it by yourself. Attorney fees range by attorney and the specifics of your case.
It is not required to have an attorney but I certainly recommend it as there is much more to these cases than just completing some forms.
No one needs a lawyer for anything but it usually is a good idea to use someone who has successfully done it a few thousand times. Nothing about US immigration is intuitive. Legal fees vary significantly depending on a variety of factors.
